We are seeking care coordinators with the skills, compassion and commitment to deliver high-quality services to people in northern New York.
Care Coordinators work with individuals who have chronic medical and/or behavioral health issues. They are responsible for linking clients with needed services and supports for successful community living.
Applicants should have strong communication skills and be able to work independently. A Bachelor’s degree in human services, as well as a driver’s license are required.

Salary for this position starts at $22-$24.20 per hour, based on experience.
